Resolution:In order to request a PBN (Personal Billing Number)/off campus calling code, you must send a completed ITS Network & Infrastructure Service Requisition Form http://www.siue.edu/its/network/facstaff/service.shtml to ITS.
To request a PBN/off campus calling code, several pieces of information are required. The individuals name (of course) and their 800 ID#, along with the person's SIUE email address. Once you have that information and the person has started employment, you should submit the request. Normally the code will be created and emailed directly to them within 24 hours.
Fax the completed form to #3333 - be sure that it is signed by the fiscal officer. After faxing it, please contact me at #3373 to verify that I have received it.
Once the completed ITS Network & Infrastructure Service Requisition Form has been received, the Caller ID/display change will take approximately a week or so to process. The PBN is usually processed within 24 hours (as stated above).
There is currently no charge to create a PBN/Off campus calling code; however, any time an off campus call is placed using this code, toll charges will be billed to the Budget Purpose Number on the service requisition.
