Blackboard Ultra (Faculty) - Tests, Forms, and Pools: Create a New Form

This article provides instructions on how to create a test or survey in Blackboard Ultra.

Creating a New Form

Instructors can create forms to obtain feedback from students in Blackboard Ultra. Forms can be used for a variety of reasons, such as to place students in a clinical experience or to learn about student interests and opinions. Forms are typically ungraded because of the type of information being collected, but grading options are available in the assessment settings.

Steps to Create a Form

  1. Navigate to your course in Blackboard Ultra.
  2. On the Course Content page, select the plus sign where you want to add the test or survey.
  3. Select Create.
  4. In the Create Item panel, select either Form under the Assessment heading.

    Under the Assessment heading, the third option is highlighted and labeld Form.

  5. A form will open. Enter a descriptive name at the top.

    At the top of the page, the default form name will appear: New Form and the date. Type here to add a more descriptive form title.

  6. Use the plus sign to add questions and content. You can add many different types of questions, including:
    • Essay Question: Set the due date, prohibit or allow late submissions, and allow class conversations.
    • Likert Scale
    • Multiple Choice
    • Reuse Questions
    • Fill submission
  7. Select Save to save your question.
  8. Click the gear icon to open the Assessment settings.
  9. Configure the assessment settings. Available settings include:
    • Details and Information: Set the due date, prohibit or allow late submissions, and allow class conversations.
    • Presentation Options: Randomize questions, present one question at a time, etc.
    • Formative Tools: Label a form used to collect information as a Formative assessment if it is not for a grade.
    • Grading and Submissions: Set the grade category, number of attempts, points, etc.
    • Additional Tools: Assign to groups
    • Description: Type a description of the assessment to be displayed on the Course Content page.
  10. The newly created form now appears on your Course Content page.
